POSITION SUMMARY:

The Enhanced Case Management (ECM) Pharmacist plays a critical role in engaging members and supporting their health journey. This position leverages clinical expertise and compassionate communication to educate members, promote treatment adherence, and improve health outcomes—particularly for those managing complex or chronic conditions requiring specialty medications. The ECM Pharmacist collaborates with providers, pharmacy vendors, and internal teams to ensure members receive appropriate, evidence-based care while empowering them to understand and follow their treatment plans.

ESSENTIAL FUNCTIONS:

  • Conduct comprehensive clinical assessments to identify member needs and opportunities for improved outcomes.
  • Utilizes nationally recognized criteria to ensure appropriate, clinical standard of care.
  • Collaborates with providers and care teams to develop individualized care plans that prioritize member understanding and engagement.
  • Monitors treatment progress and intervene when gaps in care or compliance are identified.
  • Establishes trusting relationships with members through proactive outreach to provide education on their condition and reinforce the importance of treatment compliance.
  • Addresses barrier to treatment adherence by providing resources, guidance, motivation support and referrals to vendor partners.
  • Partners with internal teams to streamline processes that enhance member experience and health results.
  • Maintains knowledge of pharmacy vendors, solutions and assistance programs to support timely access to medications and affordability solutions.
  • Work closely with Medical Management leadership and ECM team members to share insights, track progress, and implement best practices for member engagement.
  • Other duties as assigned.

EDUCATION:

Doctor of Pharmacy (PharmD) required. Current license to practice pharmacy required. Additional certifications related to diabetes care and patient education preferred.

EXPERIENCE AND SKILLS:

  • Minimum of 3 years of experience as a licensed pharmacist in a clinical or patient-facing role.
