The Care Manager Supervisor provides leadership, supervision, and support to Care Managers while maintaining a reduced caseload of children and families facing serious medical conditions, chronic illnesses, disabilities, trauma, or emotional and behavioral challenges. This role ensures the delivery of high-quality, compassionate, family-centered services through clinical oversight, case consultation, staff development, and direct service provision.
The Care Manager Supervisor is responsible for overseeing assessments, care planning, resource coordination, advocacy efforts, documentation compliance, and program outcomes. Working collaboratively with families, healthcare providers, educators, community agencies, and internal team members, the Supervisor ensures that all clients receive comprehensive support and that Care Managers are equipped to effectively address complex family needs.
This is a remote position; however, candidates must reside in New York State or surrounding States, as the role requires monthly in-person visits with clients throughout New York. Applicants must be able to meet these travel requirements.
Bachelor's degree in Social Work, Psychology, Human Services, Counseling, Public Health or a related field required. Master's degree in a related field preferred.

CLCF helps families navigate the many challenges associated with medical crises through personalized care coordination and family-centered support. Our Care Managers are experienced in working with chronic health conditions and medically fragile children and adolescents. Through education, advocacy, resource navigation, and ongoing support, we help families achieve better health outcomes and greater stability.
Minimum of 3-5 years of experience in care management, case management, social services, healthcare navigation, or a related field. Previous supervisory, team lead, or mentoring experience preferred. Experience working with children, families, and individuals facing medical, developmental, behavioral, or emotional challenges strongly preferred.
Strong leadership, coaching, and team development skills. Outstanding verbal and written communication abilities. Excellent organizational and time management skills with the ability to manage multiple priorities. Strong assessment, problem-solving, and critical-thinking skills. Demonstrated ability to provide effective advocacy and resource navigation. Ability to manage sensitive and confidential information with discretion. Proficiency in Microsoft Office Suite, Google Workspace, electronic records systems, and case management platforms. Ability to work independently while collaborating effectively within multidisciplinary teams. Knowledge of healthcare systems, social services, educational resources, and community-based support programs. Commitment to culturally responsive, trauma-informed, and family-centered care.
