Cahaba Medical Care is a Community Health Center (FQHC), accredited as a Patient-Centered Medical Home (PCMH) by the National Committee for Quality Assurance (NCQA) and Joint Commission. We have locations throughout central Alabama serving Bibb, Perry, Chilton, Shelby, Wilcox, and Jefferson Counties. Cahaba Medical Care provides high-quality health services regardless of a patient’s ability to pay by offering a discounted, sliding fee schedule based on income and family size. While this has been the mission of the clinic since its founding, our capabilities have continued to develop and grow through our designation as a nonprofit, Federally Qualified Health Center (FQHC). As an FQHC, we are stewards of annual federal funding that allows us to enhance our services to those without insurance and to those facing hardship - whether medical, social, financial or emotional.

About the Role:

The Behavioral Health Consultant/Case Manager plays a critical role in supporting individuals with behavioral health needs by providing comprehensive assessment, care coordination, and intervention services. This position aims to improve clients' mental health outcomes through personalized care plans and collaboration with multidisciplinary teams. The role involves working closely with clients, families, and healthcare providers to ensure access to appropriate resources and services. By monitoring progress and adjusting interventions as needed, the consultant helps clients achieve greater stability and quality of life. Ultimately, this position contributes to the overall effectiveness of behavioral health programs within healthcare and social assistance settings across the United States.

Minimum Qualifications:

• Bachelor’s degree in social work, psychology, counseling, or a related behavioral health field.
• At least 2 years of experience in behavioral health case management or consulting.
• Knowledge of behavioral health disorders, treatment modalities, and community resources.
• Strong communication and interpersonal skills to effectively engage with clients and healthcare professionals.
• Ability to maintain confidentiality and adhere to ethical standards in behavioral health practice.

Preferred Qualifications:

• Master’s degree in social work, counseling, psychology, or a related field.
• Licensure or certification as a Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC), or equivalent.
• Experience working within integrated healthcare settings or multidisciplinary teams.
• Familiarity with electronic health record (EHR) systems and case management software.
• Training in trauma-informed care, motivational interviewing, or other evidence-based behavioral health interventions.

Responsibilities:

• Conduct thorough behavioral health assessments to identify client needs and strengths.
• Develop, implement, and monitor individualized care plans in collaboration with clients and healthcare teams.
• Coordinate services and referrals to community resources, treatment providers, and support systems.
• Provide ongoing case management, including crisis intervention and follow-up support to ensure continuity of care.
• Document client progress and maintain accurate records in compliance with regulatory and organizational standards.
• Educate clients and families about behavioral health conditions, treatment options, and coping strategies.
• Collaborate with multidisciplinary teams to advocate for client needs and optimize treatment outcomes.
