What Your Visits Could Include
- Comprehensive nursing assessments
- Wound care and wound assessment
- Medication reconciliation and management
- Injections and other skilled nursing procedures

- Patient and caregiver education
- Identifying changes in condition
- Communicating concerns and updates with physicians
- Coordinating with PT, OT, SLP, social work, and other members of the care team
- Helping patients understand how to manage their health safely at home
- Completing timely documentation in Homecare Homebase (HCHB)

- An active Minnesota Registered Nurse (RN) license
- Strong nursing assessment and critical-thinking skills
- Previous experience in home health, hospital nursing, rehabilitation, skilled nursing, or other clinical settings
- Confidence working independently in the field
- Strong patient and family communication skills
- Excellent organization and time-management abilities
- Reliable transportation and willingness to travel throughout an assigned service area
- A nurse who takes ownership of their patients and follows through
