What You Will Do - Essential Function

The Senior Care Coordinator supports the care delivery team by coordinating complex non-clinical patient care activities and serving as a subject matter expert for care coordination processes. This role performs all Care Coordinator responsibilities while providing guidance, training support, and workflow expertise to promote efficient operations, high-quality service, and a positive patient experience for individuals with chronic and complex conditions. Success in this role requires strong organizational skills, attention to detail, effective communication, and the ability to manage multiple priorities in a collaborative healthcare environment.

• Care Coordination & Patient Support: Coordinates and supports non-clinical patient care activities, ensuring patients receive timely services, follow-up, education, and assistance in navigating care while addressing barriers and escalating concerns as needed.
• Administrative & Operational Support: Serves as a care coordination resource and team lead by providing guidance, training, workflow support, operational oversight, and process improvement to promote efficient and effective service delivery.
• Documentation & Compliance Support: Maintains accurate patient records and documentation, supports reporting and quality initiatives, and ensures compliance with organizational, regulatory, and HIPAA requirements.

Who You Are

Required Qualifications

• High School Diploma or GED.
• 3+ years' experience in healthcare, care coordination, medical office operations, patient navigation, or a related healthcare support role.
• Strong understanding of healthcare workflows, referrals, medical records management, and patient support processes.
• Working knowledge of medical terminology.
• Experience working with electronic medical record (EMR) systems and healthcare technology platforms.
• Strong organizational, problem-solving, and time-management skills.
• Ability to pass background check upon hire and throughout employment to include criminal felony & misdemeanor search, SSN validation/trace search (LEIE), education report (highest degree obtained), civil upper and lower search, 7-year employment report, federal criminal search, statewide criminal search, widescreen plus national criminal search, health care sanctions-state med (SAM), national sex offender registry, prohibited parties (OFAC) (terrorist watchlist), and a 10-Panel Drug Screen.

Working Conditions:

• High-speed internet service (cable or fiber optic) with minimum download Speed of 20 Mbps, Upload Speed of 5 Mbps, and Maximum Latency of 100 milliseconds (must be installed before starting) required.
• Frequent use of computer and phone systems.
• Must be able to constantly remain in a stationary, sitting position, communicate and exchange information with others, inspect information, perform repetitive motions with arms and fingers, interpret data, problem solve, make decisions, organize and plan, and maintain a positive and professional attitude in all situations.
• Work is performed from home with company-provided equipment. Sitting for long periods of time is expected and use of fingers and hands for typing is necessary.
• A quiet workspace with minimal background noise for calls.
