Job Summary and Responsibilities

Acentra seeks a Care Coordinator to join our growing team.

Job Summary:

The Care Coordinator supports clients by identifying and coordinating service delivery by qualified FFS providers. This role will assist members who are experiencing transition(s) of care, matching and coordinating initial appointments, arranging transportation where needed and assists in coordinating follow-up visits. This role address care coordination, education, and resource navigation without engaging in clinical evaluation, diagnosis, or treatment. The goal is to promote health and well-being through system navigation and barrier reduction, ensuring effective member and provider connection and continuity of care.

This is a fully remote position supporting Pacific Time Zone (PST) hours, with scheduling between 8:00 AM and 8:00 PM, Monday through Saturday. Bilingual candidates are preferred, including Spanish, Chinese, Russian, Farsi, and other languages.

Job Responsibilities:

• Conduct non-clinical assessments of client needs and provider preferences.
• Develop and maintain documentation provider referrals, coordination, and follow-up appointments.
• Coordinate services such as transportation and social support resources when needed.
• Collaborate with provider engagement specialists and internal and external agencies to facilitate referrals and service access.
• Document interactions and progress accurately in electronic databases, in compliance with privacy and policy standards.
• Identify gaps in provider or service delivery and assist clients in overcoming access barriers.
• Educate clients and families on available services and empower them to self-navigate systems.
• Provide culturally sensitive services and demonstrate awareness of community-specific challenges.
• Maintain regular communication with assigned clients and appropriate interdisciplinary staff.
• Participate in quality improvement initiatives and team meetings.
• Read, understand, and adhere to all corporate policies including policies related to HIPAA and its Privacy and Security Rules.

Qualifications

Required Qualifications

• Associate Degree in a related human services or health field OR will consider years of experience in lieu of a degree (total of 8 years).
• A minimum of 2+ years of experience in social work, care coordination, or community outreach.
• Understanding of local, state, and Medicaid/Medi-Cal services.
• Experience working in a high-volume inbound and outbound call environment.
• Commitment to client confidentiality and ethical practice.

Preferred Qualifications

• Case Management Certification (e.g., CCM, C-SWCM).
• Bilingual candidates - including Spanish, Chinese, Russian, Farsi, and other languages.
• Experience working with Medicaid, Medicare, or commercial health plans.
• Excellent written and verbal communication skills.
• Proficient in Microsoft Office applications and case management software.
• Ability to work independently and collaboratively within interdisciplinary teams.
• Strong organizational and time management skills.
• High level of cultural competence and respect for diversity.
• Ability to manage multiple priorities and meet deadline.

Experience in Lieu of Degree

For non-clinical roles, or when not required by the contract specifically, the Company acknowledges that practical, hands-on experience can provide skills and competencies equivalent to formal education. As such, in cases where a Bachelor's degree may be required, the Company will accept a minimum of six (6) years of directly relevant professional experience in lieu of a degree. In instances where the candidate has an Associate's degree, the Company will accept a minimum of three (3) years of directly relevant professional experience in lieu of the Bachelor's degree.
