Position Overview:

We are seeking a compassionate, organized, and computer-savvy Floater to support members receiving Enhanced Care Management (ECM) and Community Supports (CS). This role combines care coordination and housing support, helping members access healthcare, community resources, and stable housing.

This position includes remote work and regular field travel for in-person appointments and outreach. County assignments will depend on member and program needs. A real estate license is not required.

Responsibilities:

  • Manage an assigned caseload of ECM and CS members.
  • Coordinate healthcare services, referrals, and community resources.
  • Assist with housing searches, applications, supporting documents, and housing stability.
  • Communicate with healthcare providers, landlords, property managers, and community partners.
  • Conduct in-person appointments, telehealth visits, and follow-up outreach.
  • Maintain timely contact notes, care plans, and housing plans.
  • Participate in a designated monthly outreach week, generally at the beginning or toward the end of the month.
  • Organize field visits by location and communicate member needs or barriers to the supervisor.

Qualifications:

  • Must reside in California and be available for regular travel throughout the assigned counties.
  • Valid driver’s license and reliable transportation.
  • Strong computer, communication, organization, and documentation skills.
  • Ability to work independently and maintain member confidentiality.
  • Experience in case management, housing support, social services, or community outreach preferred.

Schedule:

Each week includes one 7 AM–3 PM shift, three 9 AM–5 PM shifts, and one 10 AM–6 PM shift. Specific days and monthly outreach weeks will be coordinated with the supervisor.
