Advanced Care at Home Physician - Nemours Children's Health - Home Office

Nemours Virtual Care

Nemours is searching for an Advanced Care at Home physician to deliver acute, hospital-level medical care directly to a patient's residence using a blend of virtual technology, remote monitoring, and in-person visits.

Nemours Virtual Care is a centralized, enterprise-wide model that delivers technology-enabled clinical services to support patient safety, care coordination, access, capacity optimization and acute care delivery across the health system.

Core Responsibilities

  • Patient Acquisition: Review and accept new hospital admissions or patient transfers into the home care program.
  • Daily Rounding: Conduct virtual or in-person rounds to monitor recovery, update treatment plans, and evaluate vital signs.
  • Care Coordination: Collaborate with nurses, pharmacists, case managers, and medical specialists to ensure seamless treatment.
  • Diagnostics and Treatment: Order and interpret lab tests, imaging, and medications tailored to a home setting.
  • Discharge Planning: Manage safe transitions out of the acute home program back to primary care.

Key Qualifications

  • Clinical Experience: Inpatient, hospitalist, or acute care experience.
  • Licensure: Active medical license (MD/DO)
  • Tech Skills: High comfort level using Electronic Medical Records (EMRs) and remote patient monitoring tools.
  • Autonomy: Ability to make independent clinical decisions in a non-traditional environment.

Qualifications

  • Clinical Qualifications: Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO).
  • Clinical experience in managing children with medical complexity.
  • Must be eligible for licensing in PA, NL, DE, MD and FL.
  • Direct clinical experience managing acutely ill patients with experience in providing comprehensive hospital-based care, home and community care, and familiarity with Mobile Integrated Health (MIH)/Community Paramedicine.

  • Proficiency with Epic EMR, telehealth platforms, and virtual care technologies.
