As a Senior Case Manager, you'll work with individuals experiencing chronic and acute health conditions, helping them overcome barriers to care through personalized support, education, and coordinated services. You'll build meaningful relationships with members, caregivers, healthcare providers, and community partners to develop solutions that improve health outcomes and enhance quality of life. Bachelor's degree in Social Work, Psychology, Sociology, or other Health or Human Services related discipline. 24 months experience in home and community-based services, case management, or related healthcare setting.
Certified Case Manager (CCM). The employee is allowed 1 year from hire date or promotion to Senior Case Manager to obtain the CCM certification.
One year of paid professional experience in direct service provision, care planning, and has provided services to the aging and/or disabled population. Certified in Case Management
