Responsibilities:

• Provide skilled nursing care to patients in a residential home care setting
• Assess, plan, implement, and evaluate individualized patient care plans
• Administer medications, treatments, and medical procedures as ordered by physicians
• Monitor and document patient health status, progress, and changes in condition
• Coordinate care with physicians, therapists, and other healthcare team members
• Educate patients and family members on health management, medications, and care techniques
• Ensure compliance with all state, federal, and agency regulations and standards of care

Requirements:

• Active Registered Nurse (RN) license in the state of Missouri
• Current CPR/BLS certification required
• Previous home health, residential care, or community nursing experience preferred
• Strong clinical assessment and critical thinking skills
• Excellent communication and patient education abilities
• Reliable transportation and valid driver's license
• Compassionate, self-motivated, and able to work independently
