As a Clinic Nurse (RN), you will:

  • Coordinate and manage patient care plans across healthcare providers and services.
  • Assess patients’ clinical, psychosocial, and functional needs to identify appropriate interventions.
  • Collaborate with physicians, nurses, social workers, insurance companies, and community resources.
  • Monitor patient progress, outcomes, and barriers to care while adjusting plans as needed.
  • Advocate for patients by promoting quality care, safe transitions, appropriate resource utilization, and continuity of care.

A few must-haves for Clinic Nurses:

  • Graduate of an accredited nursing program with an active RN license in Indiana
  • Minimum of 2+ years of experience as an RN Case Manager
  • Current CPR certification from the American Heart Association
  • Prefer certification in Case Management
  • Strong assessment skills, critical thinking, and the ability to work independently
  • Compassionate communicator who values collaboration with students, staff, and families
