Job Description

Under the supervision of the Nurse Manager, guided by the policies and procedures of the Ambulatory Care Center, and in collaboration with the physician and other clinical staff, the Clinical Nurse 2 is responsible for providing safe, efficient nursing care to patients in an out-patient, primary care practice. Triages patients and responds appropriately according to protocol and nursing assessment.

The Clinical Nurse 2 utilizes the nursing process (Assessment, Planning, Implementation and Evaluation) to provide safe and therapeutic clinical care. The Clinical Nurse 2 also provides clinical support to Providers and participates as part of the health care team to assure coordination and continuity of patient care. The Clinical Nurse 2 is responsible for triage on behalf of the practice both in person and through telephone and electronic communication. S/he participates in the development and implementation of educational programs and materials for patients and their family members. S/he delivers direct patient care to a variety of patient populations. All components of nursing care will be individualized to meet the patient’s physiological and psychosocial development. The Clinical Nurse 2 will play a critical role in the patient experience including optimizing clinic flow, providing in and outreach to patients and working collaboratively with the team to create a patient centered environment. S/he is responsible for daily clinical operations and maintaining a safe environment for patients and staff. The Clinical Nurse 2 consistently projects a professional and courteous demeanor, helps establish and maintain the highest service standards for the practice and complies with the Medical Center House and Telephone Standards. This employee will utilize computer skills in Apex and other UCSF computerized programs. This individual supports the standards of the policies and procedures of the practice, and is responsible for maintaining current knowledge of pharmacology and competency in modes of administration, and complies with all Ambulatory Services and Medical Center policies and procedures for infection control, safety, and administrative and clinical practice.

The Clinical Nurse 2 will be primarily located at China Basin, however must have flexibility to cover multiple UCSF medical center primary care clinic sites.

RESPONSIBILITIES

Responsible for maintaining the knowledge and abilities essential to the performance of stated behaviors. Carries out legal responsibilities within the scope of the Nurse Practice Act. Accountable to the NURSE SVC SUPV 2 for overall work performance.

Key Responsibilities

  • ASSESSMENT - Identifies the needs of patients through observations of disease symptoms, emotional state, cultural and socioeconomic background and response to medical regimen and treatment.
  • PLANNING - Plans as appropriate to the needs of patients and uses knowledge of the cognitive physical, emotional, and chronological maturation process.
  • IMPLEMENTATION / EVALUATION - Provides individualized nursing care which responds to the special needs of the patient population and demonstrates an understanding of the treatment needs required for patients.
  • COORDINATION / CONTINUITY / COMMUNICATION - Organizes nursing care based on the needs of patients, while maintaining the effective flow of patients through the clinic.
  • ADVOCACY - Provides patients and/or their families with the necessary information to make informed decisions utilizing available resources and makes referrals appropriately.
  • PATIENT EDUCATION - Participates in identifying the educational needs of all patient populations.
  • SELF-DEVELOPMENT / PROFESSIONALISM - Assumes responsibility for own career growth, recognizing the need to increase knowledge, stays abreast of current trends and developments within own area of practice, expand skills and show initiative.
  • OPERATIONS MAINTENANCE - Maintains adequate levels of clinical supplies & laundry.

QUALIFICATIONS

REQUIRED QUALIFICATIONS

  • Minimum 5 years experience as a Registered Nurse
  • Minimum of Two (2) years recent experience as a Registered Nurse in Primary Care
  • Phone triage experience
  • Recent experience placing PIV’s
  • Charge Nurse experience is required
  • Recent experience with bubble studies
  • Flexibility to orient and work at all UCSF Health Locations

PREFERRED QUALIFICATION

  • Strongly prefer experience in family medicine phone triage
  • Recent Experience in the use of Electronic Medical Record documentation.
  • BSN - Bachelor of Science, Nursing Degree
  • Ability to work competently and efficiently with Microsoft Office: Outlook, Excel, and Teams.

REQUIRED LICENSES/CERTIFICATIONS

  • Registered Nurse as defined by California Board of Registered Nursing
  • Current CA Nursing license
  • Current American Heart Association CPR Certification.
