Patient clinical manager (pcm/rn)

St. Joseph Villas Homecare and Hospice, a premier Home Health and Hospice company in Omaha, Nebraska, has an opportunity for a Patient Clinical Manager.

Job duties include:

• Manage staff compliance with regulatory expectations
• Audit patient records for ongoing monitoring of documentation compliance
• Perform accurate and timely clinical reviews to meet deadlines
• Compile, review, analyze and graph audit data
• Maintain quality data and reports
• Work with hospice staff to follow up on audit results
• Collaborate with clinical team to identify performance improvement/QAPI issues and staff educational needs
• Coordinate and execute staff education as it relates to above
