Palliative Nurse Practitioner

We are seeking a Nurse Practitioner to join our growing palliative care program. This is an opportunity to make a meaningful impact by supporting patients with complex, chronic, or serious illnesses in a collaborative, patient-centered environment.

Position Details:

This position is PRN with the possibility of growing into part time or full time.

Responsibilities:

• Conduct comprehensive palliative care visits, including assessment, diagnosis, and management of symptoms.
• Complete F2F encounters for hospice patients as needed.
• Develop and implement individualized care plans in collaboration with the interdisciplinary team.
• Prescribe medications and coordinate care for patients with advanced or chronic illnesses.
• Accurately document all encounters in the EMR.
• Communicate effectively with patients, families, and other healthcare professionals.

Requirements:

• Experience preferred but willing to train new Nurse Practitioners!
• Active Nurse Practitioner license in Wisconsin.
• Strong clinical assessment, documentation, and care planning skills preferred.
• Experience with eClinicalWorks (eCW) EMR preferred (willing to train).
• Excellent communication and teamwork abilities.
