Job Description

JOB PURPOSE:

This position is responsible for providing direct services to homeless individuals who reside at the Siena Francis Miracles Residential Treatment Center. This position is also responsible for providing in-reach, case management, mentorship and coordination of support services for clients of the Miracles Program. This position serves under the direction of the Program Manager.

ESSENTIAL JOB DUTIES:

  • Provide intensive case management services for clients of the Miracles Program to include medical, physical, emotional, psychosocial, financial assistance and housing needs.
  • Provide direct assistance to clients including, but not limited to, intake, evaluation, translation, transportation, referral, food, shelter, and clothing.
  • Assist in the development and administration of a social/human service programs for clients of the Miracles Program.
  • Utilize a strength based and trauma-informed approach to work collaboratively with clients and other treatment team members to help support clients in their treatment and recovery.
  • Assist in the collection of usable information for reporting purposes and in order to apply for grant funds.
  • Assist in the implementation of policy and procedures of the Miracles Treatment Program.
  • Complete documentation and maintain quality, up-to-date records; enter necessary information into Homeless Management Information System (HMIS).
  • Coordinate activities with other agencies such as Charles Drew Health Center, Visiting Nurse Association, and Community Alliance.
  • Participate in all assigned meetings, staff development and training as required.
  • Remain culturally sensitive and respectful in all client and staff interactions.
  • Adhere to all client confidentiality requirements & standards.
  • Consult with Program Manager as needed.

EDUCATION & EXPERIENCE REQUIREMENTS:

  • Bachelor’s degree in social work, sociology, psychology or related field is preferred; experience work in the areas of case management and provision of support services necessary.
  • Two years employment experience in social services and case management preferred.
  • Ability to incorporate diversity and culturally sensitivity to programs.
  • Excellent communication and interpersonal skills.
  • Basic working knowledge of Windows (MSWord, Excel and publisher). Good working knowledge of e-mail and Internet.
  • Positive attitude; energetic; dedicated to helping the homeless and/or chemically dependent.
