Join our compassionate and dedicated team as the Manager, HealthCare Home. In this role, you will have the opportunity to oversee the daily operations of the Healthcare Home (HCH) Team, making a difference in the lives of our clients. You will coordinate and track HCH clients, facilitating health education groups and activities that empower individuals. Your leadership will guide Nurse Care Managers, fostering a culture of collaboration and excellence. As the HCH Manager, you will manage and direct daily activities within a supportive environment, ensuring client needs are effectively met. You'll engage in various initiatives, driving quality assurance and improvement within the healthcare framework.
