Home Health Scheduler

Home Health Schedulers are responsible for assignment of all patient visits to the appropriate field staff. The Home Health Scheduler serves as a leader of customer service for all lines of business within the Agency.

What You’ll Do as a Home Health Scheduler:

  • Arranges and schedules the referrals for appropriate Home Health staff based on Clinical Manager review and guidance.
  • Contact patient/caregiver
  • Assign all ordered disciplines and task all assigned clinicians in EMR
  • Enter SOC service code and Evaluation service code on clinician’s schedules in EMR
  • Assign and schedule all discipline referral request accurately and timely based on geographic area.
  • Adherence to the Home Health Coordinator process for all Medicare, Medicaid, private insurance, and private pay referrals.
  • Coordinates the implementation of consistent customer service standards.

What You’ll Need

  • High school degree required, college degree preferred, or equivalent years of training or work experience.
  • Minimum of one-year health care/business experience in a fast-paced, customer service environment.
