Qualifications Required

Associate degree or higher in Healthcare (1+ years)

1+ years of experience in ANY of the following:

• Case Management
• EHR Coordination
• Care Management

Valid Registered Nurse (RN) in the state of Kansas (KS) preferred

Valid BSN

Overview

This role involves coordinating patient care plans to ensure quality and timely service delivery. You will assess and evaluate health care needs, focusing on positive health outcomes through coordinated and efficient care. The position requires respectful and fair treatment of patients and their families, maintaining dignity and open communication.

Assesses, plans, implements, monitors, and evaluates options and services to meet individual's health care needs and the family systems that support the patient.
