The Role:

This role involves coordinating patient care plans to ensure quality and timely service delivery. You will assess and evaluate health care needs, focusing on positive health outcomes through coordinated and efficient care. The position requires respectful and fair treatment of patients and their families, maintaining dignity and open communication.

Key Responsibilities:

• Coordinate the plan of care focusing on quality, timely delivery of services, and medical necessity of patient admission
• Assess, plan, implement, monitor, and evaluate options and services to meet individual health care needs and family support systems
• Ensure well-orchestrated, coordinated care focused on quality, efficiency, and safety
• Maintain an open and approachable manner treating others fairly and respectfully, preserving self-confidence and dignity
• Coordinates the plan of care focusing on quality, timely delivery of services, and medical necessity of a patient's admission, including what level of care is appropriate.
• Assesses, plans, implements, monitors, and evaluates options and services to meet individual’s health care needs and the family systems that support the patient.
• Care Manager seeks positive health outcomes by ensuring well-orchestrated, coordinated care focused on quality, efficiency and safety.
• This position maintains an open, approachable manner treating others fairly and respectfully preserving self-confidence and dignity as well as show regard for their opinion(s).

Required Qualifications:

• Associate degree or higher
• Healthcare (1+ years)
• 1+ years of experience in ANY of the following:
• Case Management
• EHR Coordination
• Care Management
• Valid Registered Nurse (RN) certification/license in the state of Kansas (KS)

Preferred Qualifications:

• Valid BSN certification/license
