More Than a Job. A Chance to Be Part of Someone's New Beginning. A hospital discharge isn't always the end of someone's healthcare journey. Sometimes, it's the beginning of an even more important one. Where will I recover? Who can help me navigate my next steps? Where am I going to live? How do I build a more stable future?
At Anchor Co-Living, we believe recovery and stability begin with more than a roof over someone's head. They begin with compassion, consistency, connection, and having the right support at the right time. We're looking for a compassionate, responsible, and highly organized Live-In Care Coordinator who wants to make a meaningful difference in the lives of individuals recovering from hospitalization, experiencing housing instability, or navigating complex healthcare needs.
This is a unique opportunity to live within one of our residential homes while helping create an environment where members feel supported, respected, and empowered to move forward. You'll combine clinical knowledge, care coordination, housing navigation, and everyday resident support to help members move from recovery toward independence. You won't just coordinate care. You'll be a consistent, trusted presence in a place where recovery and new beginnings happen.
Think of yourself as part care coordinator, part advocate, part problem-solver, and part steady hand. Because you'll be based within the home, you'll have the opportunity to build meaningful relationships with residents, recognize challenges early, and help create a positive and supportive living environment.
Because sometimes the most important part of recovery is knowing someone is paying attention.
A stable home is about more than four walls. It's about creating an environment where people can focus on moving forward.
Getting housed is a milestone. Staying housed is the goal.
Your role isn't simply to make a referral. It's to help make sure the connection happens.
Because compassionate care and excellent documentation go hand in hand.
We're looking for someone who understands that living where you work requires maturity, professionalism, healthy boundaries, and a genuine passion for helping others. You're someone who:
And most importantly, you believe that people deserve to be treated with dignity, especially during life's most difficult transitions.
At Anchor, success is about creating consistency, stability, and meaningful progress. Residents feel supported. Members have a reliable, compassionate point of contact and receive consistent care coordination. The home is a positive environment. Residents are treated with dignity, concerns are addressed, and shared-living expectations are supported. Recovery stays on track. Medical follow-ups, transportation, referrals, and essential services are coordinated. Housing goals move forward. Members receive appropriate support toward independent living and long-term housing stability. Documentation is always current. Check-ins, care plans, authorizations, and service notes are completed accurately and within required timeframes. Communication is proactive. Leadership and care teams receive timely updates, and concerns are escalated appropriately. Most importantly, residents are making progress toward a healthier, more stable future.
Because this is more than a traditional care coordination position. It's an opportunity to become part of a community where your presence, compassion, and professional skills can make a meaningful difference. At Anchor, you'll work at the intersection of healthcare, housing, and human connection. You'll help individuals recover from difficult circumstances, develop confidence, access resources, and work toward greater independence. And because this is a live-in opportunity, you'll have a unique perspective on the everyday challenges and victories that shape someone's journey. Sometimes making a difference means coordinating a medical appointment. Sometimes it means helping someone navigate a housing application. And sometimes it means being a steady, supportive presence during a difficult chapter of their life. Those moments matter. Your Presence Matters. Your Compassion Matters. Your Work Changes Lives. Join Anchor Co-Living and help turn recovery into stability -- one day, one connection, and one person at a time.
