The Behavioral Health Care Manager plays a vital role in bridging the gap between medical and behavioral healthcare, supporting patients with behavioral health needs, and collaborating with interdisciplinary teams to improve access, engagement, and outcomes.

Position Summary

The Behavioral Health Care Manager (BHCM) is a key member of Universal Primary Care's integrated behavioral health team, working collaboratively with primary care providers, behavioral health clinicians, nursing staff, and other healthcare professionals to coordinate and support the behavioral health needs of patients. This position focuses on patient engagement, behavioral health assessment and screening, individualized care planning, measurement-based care, brief supportive interventions, referral coordination, and ongoing follow-up for patients experiencing depression, anxiety, and other behavioral health concerns.

The BHCM serves as an essential connection between patients, primary care providers, behavioral health professionals, and community resources, helping patients overcome barriers to treatment and achieve meaningful improvements in their overall well-being.
