Position Overview
The Weekend Admissions Registered Nurse is responsible for completing home health admissions, conducting comprehensive patient assessments, initiating plans of care, and coordinating services to ensure patients receive high-quality care from the start of their home health journey. This position serves as the primary admissions nurse Friday through Monday.
Key Responsibilities
- Complete home health admissions and Start of Care (SOC) assessments in accordance with physician orders and agency policies
- Obtain and document comprehensive medical histories
- Perform physical assessments, including vital signs, mobility evaluations, and safety assessments
- Complete OASIS documentation accurately and within required timeframes
- Develop individualized plans of care based on patient needs and physician orders
- Educate patients and caregivers regarding diagnoses, medications, treatment plans, and home health services
- Coordinate care with physicians, therapists, social workers, home health aides, and other interdisciplinary team members
- Ensure compliance with all federal, state, and agency regulations, including HIPAA requirements
- Identify and communicate changes in patient condition to the appropriate healthcare providers
- Facilitate referrals for additional home health services as needed
- Maintain timely and accurate clinical documentation
Qualifications:
- Current and active Registered Nurse (RN) license in the State of Ohio
- Minimum of one year of nursing experience required; home health experience preferred
- OASIS documentation experience strongly preferred
- Experience with WellSky (Kinnser) preferred
- Strong clinical assessment and critical thinking skills
- Excellent communication and patient education abilities
- Current CPR certification
- Valid driver's license, reliable transportation, and automobile insurance
- Current TB test and physical examination in accordance with agency requirements
- Ability to attend orientation and training at the Aspire Home Healthcare of Ohio office
