🌲 Territories Include: Redmond, Terrebonne, Crooked River Ranch, and Prineville

What You'll Do
- ✅ Evaluate patients and develop individualized treatment plans
- ✅ Provide skilled physical therapy services in the home setting
- ✅ Collaborate with physicians, nurses, and interdisciplinary team members
- ✅ Educate patients and caregivers on treatment plans and home exercise programs
- ✅ Complete timely and accurate clinical documentation
Qualifications
- ✔️ Current Oregon Physical Therapist License
- ✔️ Valid Driver's License and reliable transportation
- ✔️ Home Health experience preferred, but not required
- ✔️ Strong communication, assessment, and clinical skills
