WHAT YOU'LL DO
- Complete patient admissions and assessments
- Provide skilled nursing care in the home setting
- Administer medications and perform treatments
- Address urgent patient needs, including catheter care and fall follow-up
- Develop and update individualized care plans
- Educate patients and families on health and disease management
- Coordinate care with physicians and other healthcare providers
- Document care and patient progress accurately
WHAT YOU'LL NEED
- Current Registered Nurse (RN) license in Utah
- Home health experience preferred
- Knowledge of infection control and nursing practices
- Strong assessment and clinical skills
- Excellent communication and documentation skills
- Reliable transportation, valid driver's license, and auto insurance
- Ability to work independently in the field
- Ability to lift, transfer, and assist patients as needed
