Job Requirements

This position supports the Care Transitions-Home Visit team/program. The care coordinator collaborates with the patients care team, which may include patient, caregivers, physicians, and community resources. This position will assist in the enrollment of moderate to rising risk members through, face to face interactions in the inpatient setting. This team member must meet productivity standards and complete work in a timely manner. This job utilizes a unique combination of skills, such as good oral communication, sales; ability to sell a patient on the benefits of the program and computer knowledge; be confident working in excel.

Minimum Requirements

Education

HS Diploma

Experience

Prior experience working with patients in an inpatient and/or outpatient setting.
Strong verbal and written communication skill

License/Registration/Certifications

• Must have a valid driver license, clean driving record, and able to travel locally.

Core Job Responsibilities

• Serve as the first point of contact for patients who are eligible for the Care Transition-Home Visit program.
• Conduct face to face (inpatient setting) and telephonic outreaches for the Care Transitions program to enroll patients in need of the intervention.
• Must be confident and comfortable visiting patients and their caregivers at the bedside while inpatient or in rehab and successfully engage them into the intervention.
• Independently determine program eligibility through chart reviews, predictive analytics, and deductive reasoning.
• Must have basic understanding of medical terms and an understanding of Care Transitions from the hospital setting to the outpatient setting.
• Thoroughly review chart records to submit various patient data to requesting parties.
• Be responsible for incoming calls, emails, and other forms of intake for the Care Transitions program, as well as assigning patients to the team for follow-up.
• Be confident in uploading documents and charting in the current EMR system.
• Be proficient in Microsoft office, especially Excel. Must be able to work with the basic understanding of formulas, managing worksheets, formatting cells, sorting data, etc.
• Identify opportunities for member intervention to promote compliance and improve clinical outcomes.
• Provide patients with information and referrals for community resources that may support their treatment and recovery.
• Build and maintain a positive working relationship with the patients, providers, care managers, agency representatives, supervisors, and office staff to ensure successful care coordination across all programs.
• Care Coordinator will be expected to drive to all hospital and facility locations located in Spartanburg, Cherokee and Union counties to visit patients, daily when needed.
• Will be responsible for watching for patient discharge, readmission and alerting the team of patient updates.
• Must meet productivity standards, daily, weekly, and monthly.
• Participate in program development efforts and be willing to provide input to optimize the enrollment process.
• Professionally resolve member/customer requests and concerns independently.
• Proactively collaborate with providers, community resources, and other colleagues to help members achieve their best possible outcomes.
• Must also be flexible and adapt to changes in the work environment; and can manage competing demands; and change the approach or method to best fit the situation.
• Must be able to take responsibility; keep commitments; and complete tasks on time.
• Properly handle member records to ensure compliance with patient health information applicable to the preservation, accuracy, and completeness of communication and/or retention of patient information, meeting all HIPAA regulations and the HITECH Act provisions as required by law.
• Serve as a back up to other Care Coordinators, as needed.
• All other duties as assigned.

While performing the duties of this position, the employee is regularly required to talk or hear. The employee frequently is required to use hands or finger, handle, or feel objects, tools or controls. The employee is occasionally required to stand, walk, sit, reach with hands and arms; climb or balance; and stoop, kneel, crouch, or crawl. The employee may have exposure to load noise. Must be able to sit for an extended period. The employee must occasionally lift and/or move up to 25 pounds. Specific vision abilities required by this position include close vision, distance vision, color vision, peripheral vision, and the ability to adjust focus. Employee must be able to drive a class D vehicle.
