Hours Per Week: 40
Schedule: Monday - Friday 8am - 4:30pm, 1 weekend per quarter, 1 winter and summer holiday.

RESPONSIBILITIES

• Patient Care: Conduct initial assessments; establish plan of care and provide prescribed treatments and demonstrated nursing abilities; coordinate the delivery of 24-hour care;
• Communication: Counsel and guide individuals and families towards self-help in recognition and solution of physical, emotional and environmental problems
• Evaluation & Documentation: Conduct reassessment of each patient on a regular basis; maintain complete and accurate clinical documentation; supervise and serve as a resource to all home care staff

REQUIRED QUALIFICATIONS:

• License and current registration to practice as Registered Professional Nurse in New York State
• Current NYS Driver’s license

PREFERRED QUALIFICATIONS:

• Bachelors degree of Science in Nursing preferred.
• 6 months- 1 year experience conducting UAS-NY Community Assessment preferred.

EDUCATION:

• AS: Nursing (Required)

LICENSES / CERTIFICATIONS:

• BLS - Basic Life Support - American Heart Association (AHA), RN - Registered Nurse - NYS Office of Professions

PHYSICAL REQUIREMENTS:

Drive - Clinical - Roles in this category require frequent driving combined with mobile clinical care across settings such as emergency response, mobile clinics, and home health environments. Staff must repeatedly enter and exit vehicles, occasionally lift and carry equipment weighing up to 100 lbs. or more, and perform care within patients’ homes or other confined, unpredictable, or uncontrolled environments. These positions demand strong fine and gross motor control, firm grasping, and heightened visual and auditory awareness to ensure safe and effective care delivery. Emotional and cognitive demands are substantial due to rapid decision‑making, direct exposure to critical or sensitive situations, and independently managing patient care in the field or home setting. Environmental exposures may include traffic hazards, infectious agents, extreme temperatures, variable home conditions, and hazardous materials. Valid licensure, physical readiness, and flexibility for extended or on‑call schedules are typically required.

PAY RANGE:

$75,733.84 - $101,014.16

The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components. Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.
