What You'll Do
- Evaluate patients' functional abilities, mobility, safety awareness, and home environments.
- Develop individualized treatment plans focused on maximizing independence and quality of life.
- Provide therapeutic interventions to improve activities of daily living (ADLs) and instrumental activities of daily living (IADLs).
- Assess and recommend adaptive equipment, home modifications, and safety strategies.
- Train patients, caregivers, and families on techniques that support successful home management.
- Collaborate with physicians, nurses, physical therapists, speech therapists, and other healthcare professionals.
- Complete accurate and timely clinical documentation in accordance with agency standards.
Qualifications
- Graduate of an accredited Occupational Therapy program.
- Current Oregon Occupational Therapist license.
- Current CPR certification or ability to obtain.
- Valid driver's license, reliable transportation, and automobile insurance.
- Home Health experience preferred but not required.
- Strong clinical judgment, organizational skills, and communication abilities.
