Position Summary

This position serves as a coordinator of a multidisciplinary team. The team provides continuity of program services in an ethical, legal and moral manner within a safe and therapeutic environment, consistent with applicable regulatory and accreditation standards. Participate as a team member in the delivery of addiction and/or mental health treatment services to clients and their families and facilitate the issues of recovery into their daily living situations.

Qualifications:

• Master's Degree in Psychology, Counseling, Social Work, or other related field.
• Must qualify as a QMHP as defined by the Illinois Department of Mental Health Rule 132.
• Previous supervisory experience preferred
• Computer proficiency required.
• Ability to navigate and document in electronic healthcare records required.
• Ability to accurately complete required documentation within the time frames
• Valid driver's license and ability to meet Agency insurance requirements

Responsibilities:

1. Work hours prescribed and any additional hours deemed necessary for the responsibilities assigned.
2. Ensure that the client service records are maintained consistent with required rules and regulations.
3. Become competent in EHR and function as a trainer.
4. Supervise assigned staff, including orientation, training, regular supervisory meetings and/or feedback, scheduled evaluations and disciplinary interventions as required.
5. Maintain communications with all staff (e.g. admissions, and discharge plans).
6. Assume on call responsibilities per schedule or when designated.
7. Present workshops, lectures and training to clients, families, and staff and community groups.
8. Assist in treatment plan and assessment creation.
9. Provide consultation to treatment team members to assist in assuring that client care needs are met.
10. Identify and develop community treatment partners to assist in providing a comprehensive care program for designated clients.
11. Assure ongoing communication and collaboration with community treatment providers once relationships are established.
12. Provide case management and group coverage when needed.
13. Ensure that a firm referral is established for each patient which includes date and time - supervisor role (Pathways)
14. Act as point person when assigned
15. Represent the department in Pl committees and organizational initiatives as determined appropriate by leadership.
16. Knowledge of psychotropic medications and ability to identify adverse effects of medications.
17. Become familiar with all related state and county contracts, and complete monthly reports as required.
18. Serve as a member of the department and participate in all team meetings and activities.
19. Exercise confidentiality in keeping with the Code of Conduct and with the framework of the law.
20. Deliver exceptional customer service consistently to every customer.
21. Serve as a role model for other staff, clients and customers and demonstrate positive guest relations in representing Rosecrance.
22. Assume other related responsibilities as assigned by management.

• Hours: Monday- Friday 8:30am-5:00pm

• Starting Pay: $74,400/ year (based on education, experience, and credentials)
