Home Health Clinical Team Specialist – Therapy
Remote | PRN |

Use your home health expertise in a flexible, remote role that supports therapy teams throughout the Northeast. The Clinical Team Specialist provides clinical oversight and documentation support during leadership PTO, periods of increased volume, and the continued growth of new branches. This position will primarily review therapy-only documentation, help maintain regulatory compliance, and partner with clinical leaders to improve quality outcomes.

What You’ll Need

• Current, unrestricted Occupational Therapist, Physical Therapist, OR Registered Nurse license in Massachusetts
• Dual licensure in Massachusetts and Connecticut preferred, but not required
• Minimum of 2 years of home health or hospice experience
• Advanced proficiency in OASIS documentation and review; OASIS certification or super-user experience preferred.
• Advanced Homecare Homebase experience; super-user experience strongly preferred
• Strong understanding of therapy documentation, plans of care, and regulatory requirements
• Ability to learn and navigate systems such as Homecare Homebase, Workday, and Microsoft Office
• Availability to occasionally travel to a field office for audits or other operational needs
• Dependable vehicle, valid driver’s license, and current auto insurance in accordance with state requirements
• Sit, stand, bend, lift and move intermittently and be able to lift 50–100 lbs.

What You’ll Do

• Review therapy-only Start of Care, Resumption of Care, Recertification, and Discharge OASIS documentation
• Review and approve therapy plans of care and proposed changes for clinical appropriateness
• Ensure documentation is accurate, complete, timely, and compliant with state and federal requirements
• Monitor required visits and documentation in Homecare Homebase
• Provide coverage during therapy manager PTO and assist branches experiencing documentation backlogs
• Support new and growing branches throughout the Northeast
• Collaborate with clinical leadership on HHCAHPS and TPS improvement initiatives
• Assist with audits, quality reviews, and QAPI activities
• Provide occasional on-site support when needed
• Complete additional projects and responsibilities as assigned
