Location: Remote/Field-Based (Candidates must reside in Arkansas)

Department: Care Coordination

Reports To: Care Coordination Supervisor

Employment Status: Full-time

FLSA Status: Non-exempt

Our Shared Future

At Arkansas Provider Coalition (APC), we believe effective care coordination goes beyond completing paperwork. Care Coordinators serve as advocates, problem-solvers, resource navigators, and trusted partners for members and their families.

As a Care Coordinator I, you will help support the overall health, safety, and well-being of assigned members while coordinating services, navigating complex systems, advocating for individuals and families, and connecting members to the resources they need.

The ideal candidate brings experience working with individuals with intellectual and developmental disabilities (IDD), behavioral health needs, substance use disorders, Medicaid services, waiver programs, foster care, community-based services, or other complex populations.

We especially value candidates with backgrounds as Qualified Behavioral Health Providers (QBHPs), Licensed Practical Nurses (LPNs), case managers, behavioral health professionals, or professionals with experience working with Arkansas state agencies such as DHS or DCFS and Arkansas Medicaid waiver programs.

If you have experience navigating complex systems, coordinating services, advocating for individuals and families, and connecting people to the resources they need, we encourage you to apply.

You and Your Job

As a Care Coordinator I, you will be responsible for coordinating services and supporting the overall health, safety, and well-being of assigned members.

You will work with members, families, guardians, providers, state agencies, community organizations, and other members of the care team to develop and maintain person-centered service plans, identify barriers to care, coordinate services, and connect members with appropriate resources.

You will also maintain required documentation, monitor member needs and performance requirements, support compliance with applicable regulations, and effectively manage a field-based schedule while balancing member needs, appointments, documentation requirements, and regulatory deadlines.

Job Responsibilities

Person-Centered Care Coordination

  • Develop, document, evaluate, monitor, and modify assigned members’ annual Person-Centered Service Plans (PCSPs) based on member preferences, needs, strengths, goals, and choices.
  • Coordinate services across multiple areas, which may include behavioral health, community resources, crisis services, IDD/Waiver services, financial resources, mental health, physical health, and substance use services.
  • Serve as an advocate and consistent point of contact for members and their families, guardians, providers, and other members of the member’s circle of support.
  • Assess barriers to care and identify appropriate resources, services, and community support.
  • Help members understand and exercise choices regarding available services and support.
  • Monitor service delivery and member needs and make appropriate modifications to the PCSP when circumstances, needs, or member preferences change.

Member & Family Engagement

  • Maintain regular contact with assigned members, families/guardians, providers, community resources, stakeholders, and other professionals involved in the member’s care.
  • Conduct required in-person member visits, including at least quarterly visits and more frequent contact for complex cases.
  • Provide telephone and virtual outreach using company-provided technology.
  • Build trusting relationships with members and families while maintaining appropriate professional boundaries.
  • Provide education, support, advocacy, and assistance in navigating healthcare and community systems.
  • Identify barriers affecting a member’s ability to access services and work collaboratively to resolve those barriers.

Collaboration & Care Coordination

  • Collaborate with healthcare providers, behavioral health professionals, waiver providers, community organizations, state agencies, families, guardians, and other members of the care team.
  • Coordinate services across healthcare, behavioral health, IDD, waiver, social service, and community systems.
  • Connect members with appropriate community and state resources.
  • Participate in team meetings, case staffing, and other care coordination activities.
  • Communicate effectively with internal and external stakeholders to promote continuity and coordination of care.
  • Support members during changes in services, providers, living situations, or other circumstances that may affect their health and safety.

Documentation, Compliance & Quality

  • Document care coordination activities, member contacts, assessments, referrals, and service plans in the electronic medical record (EMR) within required timeframes.
  • Maintain accurate and timely records in accordance with APC policies, contractual requirements, HIPAA, and applicable state and federal regulations.
  • Maintain compliance with Conflict-Free Case Management requirements and applicable Arkansas PASSE, Medicaid, Arkansas Insurance Department, and other regulatory standards.
  • Monitor assigned member metrics and performance requirements.
  • Protect the confidentiality, privacy, and security of member information and Protected Health Information (PHI).
  • Remain current on regulatory changes, community resources, Medicaid requirements, and care coordination practices.
  • Identify opportunities to improve processes and member outcomes and participate in continuous quality improvement efforts.

Technology & Field-Based Work

  • Utilize a company-issued laptop, Microsoft 365 applications, smartphone, EMR, communication platforms, and other technology required to perform essential job functions.
  • Maintain a secure home office environment with reliable internet service and appropriate safeguards for confidential information.
  • Travel regularly to member homes, provider offices, community locations, meetings, trainings, and other assigned locations.
  • Effectively manage a field-based schedule while balancing member needs, documentation requirements, appointments, and regulatory deadlines.
