About the Role

We are seeking a compassionate, patient-focused Bilingual Nurse Case Manager to support patients, healthcare providers, and clinic partners across Québec through a specialized Patient Support Program (PSP).

As a key member of the care team, you will help patients navigate their treatment journey by providing education, clinical support, care coordination, and ongoing follow-up. Working closely with physicians, clinics, pharmacies, and internal stakeholders, you will help facilitate timely access to treatment while delivering an exceptional patient experience.

This is a part-time position averaging 18.75 hours per week. Scheduled hours must fall within the program's operating hours of Monday through Friday, 8:00 a.m. to 8:00 p.m. EST.

What You'll Do:

• Serve as a primary point of contact for patients enrolled in the Patient Support Program.
• Provide education regarding prescribed therapies, treatment expectations, medication administration, and adherence strategies.
• Support patient enrollment, onboarding, and ongoing engagement activities.
• Coordinate care and communicate with physicians, clinic staff, specialty pharmacies, and other healthcare professionals throughout Québec.
• Assist patients in navigating access to treatment, reimbursement programs, and medication support services.
• Conduct patient assessments and follow-up activities in accordance with program requirements.
• Monitor patient progress and provide ongoing support throughout the treatment journey.
• Maintain accurate and timely documentation within program systems.
• Identify and escalate clinical or operational concerns as appropriate.
• Contribute to quality improvement initiatives and program performance objectives.
• Build and maintain collaborative relationships with healthcare providers, clinics, and stakeholders across Québec.

Basic Qualifications:

• Active and unrestricted nursing license in Québec (OIIQ registration required) as a Registered Nurse (RN), Licensed Practical Nurse (LPN), or equivalent nursing designation recognized in Québec.
• Fluency in both French and English, with excellent verbal and written communication skills in both languages.
• Strong clinical assessment, patient education, and care coordination skills.
• Proficiency with Microsoft Office applications and electronic documentation systems.
• Ability to work 18.75 hours per week within program operating hours of 8:00 a.m. to 8:00 p.m. EST, Monday through Friday.
• Ability to travel to clinics and healthcare sites throughout Québec as required.
• Strong organizational skills, attention to detail, and ability to manage multiple priorities.

Preferred Qualifications:

• Experience working within a Patient Support Program (PSP).
• Previous Nurse Case Management experience.
• Experience supporting patients receiving specialty medications, biologics, or complex therapies.
• Knowledge of reimbursement, patient access, and specialty pharmacy processes.
• Experience providing patient education and clinical support in both English and French.
• Demonstrated ability to collaborate with physicians, specialty clinics, pharmacies, and multidisciplinary healthcare teams.
• Experience supporting patients with chronic disease management and treatment adherence.
• Additional active nursing licenses in other Canadian provinces.

Travel, Work Environment & Physical Requirements:

• Regular travel to Québec clinics, healthcare facilities, and stakeholder meetings across the province.
• Valid driver's license and access to reliable transportation required.
• Ability to work independently in a remote environment while maintaining a high level of organization and responsiveness.
• Occasional lifting and transportation of educational or program materials may be required.

The pay range shown below is aligned with McKesson's pay philosophy, and pay will always be compliant with any applicable regulations.
