Schedule: 7:00 am - 3:00 pm (Monday – Friday)

An established, mission-driven healthcare organization in Connecticut is seeking a seasoned, clinically strong Unit Manager (RN) to oversee clinical and operational standards across two downstairs long-term care / skilled nursing units. Reporting directly to the Director of Nursing (DON), the Unit Manager carries 24/7 clinical and administrative accountability for their designated units.

Working a primary schedule of Monday through Friday (7:00 AM – 3:00 PM), you will lead unit nursing staff, drive clinical quality standards, direct resident care planning, manage family/physician relations, and maintain strict state and federal regulatory compliance across your caseload.

Key Responsibilities

• 24/7 Unit Operational Command: Hold comprehensive clinical and administrative management responsibility for two downstairs resident care units.
• Clinical Quality & Survey Readiness: Lead daily unit operations to ensure continuous compliance with Connecticut Department of Public Health (DPH) regulations, CMS guidelines, and QAPI performance metrics.
• Interdisciplinary Care Planning: Oversee comprehensive resident care plans, attending physician rounds, MDS assessments, skin/wound management programs, and hospital readmission prevention strategies.
• Unit Staff Leadership & Supervision: Manage, evaluate, and mentor floor RNs, LPNs, and CNAs; collaborate closely with the Daytime RN Desk Nurse / Assistant Unit Manager to balance daily unit workflows.
• Physician & Family Relations: Serve as the primary point of escalation for attending physicians, specialty providers, residents, and family members regarding clinical care and treatment plans.
• Incident Management & Auditing: Conduct root-cause analyses for fall incidents, medication errors, or skin integrity changes; implement corrective action plans and required regulatory documentation.

Qualifications & Requirements

• Licensure: Current, active, and unrestricted Registered Nurse (RN) license in the State of Connecticut required.
• LTC Management Experience: Minimum of 3+ years of clinical nursing experience in a skilled nursing facility (SNF) or long-term care (LTC) setting, with at least 1 to 2 years in a formal leadership role (e.g., Unit Manager, Assistant DON, or Charge Nurse).
• Clinical Depth: Deep expertise in LTC clinical regulations, infection control, wound management, care planning processes, and DPH survey preparation.
• EMR Proficiency: Advanced proficiency with Electronic Medical Record systems (e.g., PointClickCare) for care management, auditing, and physician order oversight.
• Leadership & Communication: Exceptional leadership presence with proven ability to coach floor staff, communicate clearly with families, and collaborate effectively with interdisciplinary teams.

Note on Compensation: The posted range represents base rates for specialized nursing and supervisory positions. Final pay will be determined during the interview process based on clinical background, leadership experience.
