Rn case manager, vna

Monday Friday 8am -4:30 pm with weekend rotation of 1 /4 weekend requirement. Home Health experience required. Under the direction of the Clinical Team Manager and within established organizational and departmental policies and procedures, responsible for providing skilled care and case management to patients/families in accordance with the State Nurse Practice Act and accepted professional standards of practice. The RN case manager provides services utilizing nursing theory and processes, communicates/documents observations and assessments, promotes and maintains health of individuals through direct care, teaching, counseling and appropriate rehabilitative measures.
