RN Care Manager Overview:
As an RN Care Manager at Good Shepherd Community Clinic, you’ll help patients navigate the parts of healthcare that can be difficult to manage on their own. You’ll work with patients over time, using your clinical knowledge to assess their health, understand the challenges they are facing, and help them take the next steps in their care. You’ll develop meaningful relationships with patients as you provide ongoing support, working closely with our providers and care team to help patients understand their care and navigate their health needs.
What You'll Do:
- Assess patients’ needs: Evaluate each patient’s health and unique circumstances, develop and monitor individualized care plans, and coordinate appropriate services.
- Provide clinical education and guidance: Educate patients on their health conditions, medications, treatment plans, and self-management strategies to help them manage their health effectively.
- Coordinate transitions of care: Support patients following hospital or emergency department visits by completing medication reconciliation, coordinating with teams and specialists, and arranging follow-up care and referrals.
- Build relationships over time: Develop meaningful relationships with patients, understand their individual circumstances and challenges, and provide consistent support as they work toward their health goals.
- Advocate for patient needs: Identify needs and barriers that may affect a patient’s care and regularly communicate progress to the care team while ensuring care strategies remain aligned across the team.
- Monitor patient progress: Use care pathways and tools to guide interventions, document care management activities, and track patient progress and outcomes in the EHR.
What We’re Looking For:
- Strong clinical judgment and care planning skills.
- Clear, compassionate communication, including the ability to navigate sensitive or emotionally complex situations.
- Proficiency with EHR systems and care management tools.
- Understanding of social determinants of health and their impact on access to care.
Education & Experience:
- Associate or bachelor’s degree in nursing from an accredited program.
- Current, unrestricted Oklahoma RN license in good standing.
Preferred:
- Minimum three (3) years of clinical nursing experience with preference for case management, ambulatory or chronic care.
- Experience in a community health center, FQHC, PCMH, public health setting, or similar environment.
- Case management certification, such as Certified Case Manager (CCM) or Accredited Case Manager (ACM), or a similarly recognized case management credential.
