Registered Nurse – Home Health

Schedule 8-hour shifts

Assignment Details

• Guaranteed Hours: 40
• Contract Weeks: 34

Requirements

• Active RN license
• Minimum of two years nursing experience
• One year of home health experience
• OASIS and HomeCare HomeBase experience preferred
• Valid state driver's license and automobile liability insurance
• Ability to drive in all weather conditions

Responsibilities

• Ensure individualized client care plans are executed and reassessed as necessary
• Complete and review documentation for patient transfers to inpatient facilities
• Process evaluations and verify data accuracy
• Manage referral intake and coordinate assessment visits
• Lead case conferences and facilitate care coordination
• Document actions and follow up on vital sign alerts and wound score deviations
• May perform patient visits and participate in on-call rotation
