About the Position:

Under the direction of the Director of Physician Services, the Program Manager, PCMH is responsible for coordinating, implementing, and sustaining the Patient-Centered Medical Home (PCMH) program across participating primary care clinics. This position oversees centralized care management operations, supervises assigned program staff, coordinates interdisciplinary collaboration, and supports compliance with NCQA recognition standards. The Program Manager leads program development, staff education, workflow implementation, performance monitoring, quality improvement, and reporting activities. Collaborates with clinical leadership, clinic management, information technology, quality, and external partners to support patient-centered care, improve healthcare access and outcomes, and ensure the program meets established organizational and grant objectives.

About the PCMH Program:

CPH is introducing a new Patient-Centered Medical Home initiative focused on strengthening how patients experience and access primary care. The PCH model brings members of the healthcare team together to improve communication, coordinate services, and help patients access the care and resources they need. The Program is designed to support the whole person by strengthening connections between patients, their providers, and community resources. As part of this new initiative, you'll have the opportunity to help build and support a program that makes a meaningful difference for patients and the community we serve in Soldotna and the surrounding Kenai Peninsula.

REQUIREMENTS:

• Associate’s degree in Healthcare Administration, Business Administration, Project Management, Public Health, or a related field.
• Relevant professional experience may be substituted for required education on a year-for-year basis
• 2 years of experience in a clinical or hospital setting
• 1 year of experience in a progressively responsible role in a healthcare setting
• Valid State of Alaska driver's license
