Wellspring is seeking a full-time Program Manager for FACT who oversees the safety, support and wellbeing of program clients and manages and supervises a multidisciplinary team. The role leads daily care coordination, ensures evidence-based Integrated Dual Diagnosis Treatment and Housing First services, and oversees outreach, assessments, treatment documentation, referrals, advocacy and staff supervision. The Program Manager collaborates with community partners to coordinate referrals, supportive services, outreach and required program data. The following areas are key responsibilities include but are not limited to:
Minimum of two years of progressively responsible experience providing behavioral health services to adults with serious mental illness (SMI), including experience working with individuals with co-occurring mental health and substance use disorders. Demonstrated supervisory, program, or team leadership is required. Experience with community-based behavioral health services, housing or homelessness services, interdisciplinary treatment teams, Medicaid documentation, and/or managed care authorization processes is strongly preferred. Certified Social Worker (CSW) is required; Licensed Clinical Social Worker (LCSW) preferred.
$60-$65K
