Our 915-bed nonprofit, acute, tertiary, teaching Level I trauma center hospital is in the heart of Los Angeles. The Pharmacy Department has over 500 team members who are dedicated on providing both inpatient and outpatient pharmacy services, ensuring that patients receive high-quality care tailored to their needs.

This is a Monday - Friday, onsite role.

Join Cedars-Sinai Medical Center in advancing safe, effective, and patient-centered diabetes care across the health system. As the Diabetes Stewardship Pharmacist, you will serve as a clinical and strategic pharmacy leader in advancing a comprehensive diabetes stewardship program within the medical center. Working closely with physicians, pharmacists, nurses, diabetes educators, informatics specialists, and other members of the interdisciplinary diabetes team, you will help reduce patient harm related to insulin administration, optimize institutional glucometric outcomes, and promote evidence-based pharmacotherapy for hospitalized patients with diabetes.

In this collaborative role, you will help design and implement a real-time surveillance tool embedded within the electronic health record to identify patients experiencing glycemic excursions, support timely care triage, and enhance delivery of individualized pharmacotherapeutic recommendations. You will provide clinical support to frontline pharmacy colleagues through patient-specific medication reviews, therapy optimization, and management of complex inpatient diabetes cases. You will also help coordinate medication use evaluations, quality improvement initiatives, clinical audits, and outcomes reporting designed to improve glycemic management, medication safety, and patient care. Your contributions will help establish consistent, measurable, and evidence-based diabetes care across Cedars-Sinai while advancing the organization’s commitment to patient safety, clinical excellence, education, and innovation.

QUALIFICATIONS

EDUCATIONAL REQUIREMENTS:

• Pharm.D. degree from an accredited School of Pharmacy in the USA
• Completion of a PGY1 and/or PGY2 residency or equivalent practice experience

LICENSE/CERTIFICATION/REGISTRATION REQUIREMENTS:

• Current pharmacist licensure in the State of California.
• BLS certification upon hire
• Board Certification in Pharmacotherapy (BCPS) or in specialty area is preferred

WHAT ELSE ARE WE LOOKING FOR?

• At least three years or equivalent experience as an inpatient pharmacist in a major medical center
• Has BC-ADM (Board Certified - Advanced Diabetes Management) certification, or ability to obtain within three years of hire
• Passionate about improving the safety, quality, and effectiveness of inpatient diabetes care, particularly insulin use and glycemic management
• Possesses good interpersonal skills and the ability to communicate complex clinical, pharmacotherapeutic, and operational information clearly to diverse audiences
• Demonstrates strong clinical judgment and critical-thinking skills, including the ability to evaluate glycemic data and provide individualized, evidence-based recommendations for complex hospitalized patients
• Has an interest in using electronic health record tools, data analytics, and real-time surveillance to identify glycemic excursions and support timely clinical intervention
• Demonstrates a commitment to quality and process improvement, outcomes measurement, and translating evidence into sustainable clinical practice
• Exhibits excellent verbal and written communication skills
• Demonstrates ability to prioritize projects to meet goals
