Clinical Pharmacist

Schedule: Full-time position with shifts Monday through Friday between 7:30 AM and 5:00 PM. Every fourth weekend requires half days on Saturday and Sunday.

Assignment Details

This role has no on-call requirement. Holiday coverage is required as needed.

Requirements

• Bachelor's degree in Pharmacy
• Licensed Pharmacist
• Basic Cardiac Life Support (BLS) certification from the American Heart Association (AHA)

Preferred: Doctor of Pharmacy (Pharm.D.), completion of an accredited postgraduate training program (PGY-1 and/or PGY-2) residency program, Pediatric Advanced Life Support (PALS) certification, Advanced Cardiac Life Support (ACLS) certification, Neonatal Resuscitation Program (NRP) certification.

Responsibilities

• Provide patient-focused pharmaceutical care by reviewing and monitoring medication therapy.
• Verify medication orders and make dosing adjustments as necessary.
• Conduct medication reconciliation and provide patient education.
• Participate in multidisciplinary patient care rounds.
• Support medication safety and performance improvement initiatives.
• Provide education to staff and assist with departmental coverage as needed.
