Job Description

Clinical Pharmacist

Schedule: Full-time position with shifts Monday to Friday between 7:30 AM and 5:00 PM. Every fourth weekend will require working half days on Saturday and Sunday.

Requirements

Bachelor's degree in Pharmacy
Licensed Pharmacist
Basic Cardiac Life Support (BLS)

Responsibilities

Provide patient-focused pharmaceutical care
Review and monitor medication therapy
Verify medication orders and make dosing adjustments
Conduct medication reconciliation and provide patient education
Participate in multidisciplinary patient care rounds
Support medication safety and performance improvement initiatives
