Patient safety associate - central monitoring unit

The Patient Safety Associate supports the nursing team by providing continuous, direct observation of assigned patients to promote safety and reduce risk. The Patient Safety Associate is accountable to the registered nurse or charge nurse coordinating unit activities and receives assignments through the staffing office. This role provides high quality service to patients, families, and interdisciplinary team members while maintaining clear, professional communication and strict adherence to patient safety protocols.

Receive and review patient assignment information from the team lead, including observation and handoff documentation.

Introduce self to assigned patients and clearly explain observation responsibilities in a respectful and supportive manner.

Maintain continuous observation of assigned patients and remain in the patient room unless relieved or approved by the registered nurse.

Engage patients in appropriate activities to support comfort, orientation, and emotional well being during the shift.

Monitor patient condition, behavior, and mood, and immediately report any changes or emergencies to the registered nurse.

Maintain unobstructed visual observation of the patient at all times, including during restroom use and patient transport for testing procedures.

Assess the patient environment for safety risks and ensure the room is free of hazardous items, clutter, or tripping hazards.

Assist with patient ambulation and transfers with approval and direction from the registered nurse or charge nurse.

Prepare meal trays for patients who are able to self feed. Document patient observations accurately and timely in accordance with established protocols.

High school diploma or G.E.D and Florida CNA certification.

Knowledge of patient safety principles related to continuous observation and risk prevention.

Ability to recognize and promptly escalate changes in patient condition, behavior, or safety status.

Skill in maintaining focused visual observation without distraction for prolonged periods.

Ability to follow clinical instructions, safety protocols, and unit specific procedures with consistency.

Knowledge of environmental safety standards in patient care settings.

Ability to communicate clearly, calmly, and professionally with patients experiencing distress or confusion.

Skill in documenting observations accurately using standardized forms or electronic systems.

Ability to maintain patient dignity, privacy, and personal boundaries during observation activities.

Knowledge of basic patient mobility principles and safe assistance techniques under clinical guidance.
