Summary
The Outreach and Navigation Manager provides strategic leadership for NOHN's outreach, community engagement, patient navigation, and social needs resource programs. As a key leader within a Federally Qualified Health Center (FQHC), this position develops and implements initiatives that improve access to care, reduce barriers to health services, strengthen community partnerships, and support fair access to healthcare resources for underserved and vulnerable populations.
Responsibilities
- Develop, implement, and evaluate outreach, patient navigation, and community engagement strategies.
- Provide leadership and oversight for outreach staff, Community Health Workers, Resource Navigators, and other assigned personnel.
- Establish annual goals, performance metrics, and quality improvement initiatives.
- Develop and maintain collaborative relationships with healthcare organizations and community-based organizations.
- Lead organizational efforts related to identifying and addressing social drivers of health.
- Develop initiatives that improve patient access to primary care and preventive health services.
- Serve as the primary point of contact for community organizations interested in partnering with NOHN's Mobile Health Clinic.
- Collaborate with Quality, Compliance, and Leadership teams to support Community Health Needs Assessments.
- Recruit, train, mentor, coach, and evaluate assigned staff.
Qualifications
Education Required: Bachelor's degree in Public Health, Healthcare Administration, Social Work, Human Services, Community Health, Business Administration, or a closely related field.
Preferred: Master's degree in Healthcare Administration, Public Health, Business Administration, Social Work, or related field.
Experience Required: Minimum of five (5) years of progressively responsible experience in healthcare, public health, community health, social services, care coordination, population health, community outreach, or related field.
Preferred: Experience in an FQHC, Community Health Center, rural health setting, public health agency, or integrated healthcare environment.
