Patient Care Coordinator – Structural Heart Program

Capital Cardiology Associates is seeking a highly organized, detail-oriented, and dependable Patient Care Coordinator to join our Structural Heart Program.

If you’re someone who thrives in a busy office environment, enjoys helping patients navigate the healthcare system, and takes pride in getting the details right, we’d love to hear from you.

This is a full-time, Monday–Friday administrative position supporting our specialized cardiology team and helping coordinate care for patients with structural heart conditions.

You’ll play an important role in keeping the day-to-day operations of the program running smoothly. This position involves frequent communication with patients, physicians, hospitals, and medical insurance companies, as well as extensive use of electronic medical records and other computer systems.

  • Provide administrative support for the Structural Heart Program and care team
  • Communicate frequently with patients by phone and assist with questions and care coordination
  • Work directly with medical insurance companies regarding patient services, coverage, and related requirements
  • Coordinate and facilitate patient appointments, testing, procedures, and other aspects of care
  • Maintain accurate and timely documentation in the electronic medical record
  • Communicate with physicians, clinical staff, hospitals, and other healthcare partners
  • Track details and follow up on outstanding items to ensure nothing falls through the cracks
  • Manage multiple priorities while maintaining a high level of accuracy and organization
  • Provide compassionate, professional service to patients throughout their care journey
  • Perform other administrative duties necessary to support the program

The ideal candidate is:

  • Highly organized and able to manage multiple priorities
  • Extremely detail-oriented and meticulous with documentation and follow-up
  • Comfortable spending much of the day on the phone and computer
  • Professional, friendly, and confident when communicating with patients and healthcare professionals
  • Experienced working with medical insurance companies
  • Comfortable learning and navigating an electronic medical record (EMR) system
  • Able to work independently while also being an effective member of a team
  • Dependable, responsive, and committed to following tasks through to completion

Previous administrative, medical office, patient coordination, or healthcare experience is strongly preferred. Experience working with insurance companies is a significant plus.

This is an opportunity to become part of a specialized cardiology program where your organizational skills and attention to detail directly contribute to a positive patient experience and help our clinical team provide excellent care.
