The Knight Cancer Institute Center for Hematologic Malignancies (CHM) outpatient clinic provides care for multiple different types of hematologic malignancies including Lymphomas, Leukemias and Multiple Myeloma.

We are seeking an Advanced Practice Provider (APP) who will provide acute and long-term care to patients who have undergone an allogeneic hematopoietic cell transplant (HCT) to join our dedicated Bone Marrow Transplant (BMT) Program.

Function/Duties of Position

Position Summary: OHSU classification: Clinical Associate APP, represented by the Oregon Nurses Association & Advance Practice United, subject to the active collective bargaining agreement. This Clinical Associate-APP provides care in conjunction with a team comprised of physicians, APPs, RN coordinators, and other allied health professionals. APPs may practice both independently or in a shared-care model with physicians, as determined by the needs of the team. Additionally, this APP will perform procedures and may intermittently provide oversight to the infusion unit.

The ideal candidate will thrive in a fast-paced, team-oriented environment and demonstrate clinical knowledge and oncologic experience to support a complex patient population.

The Clinical Associate-APP delivers care aligned with a defined scope of practice in Outpatient Adult Hematology & Medical Oncology, with work expectations operationalized through departmental workflows, productivity standards, and performance expectations consistent with the collective bargaining agreement and institutional policies.

This role is structured to meet contract-defined work expectations, inclusive of daytime coverage, aligned with the clinical coverage model and patient care needs of the service. A fixed schedule will be determined at the time of hire and may include evening, weekend, and holiday coverage.

Center for Health & Healing, Building 2, multiple floors (primary location)
Center for Health & Healing, Building 1 (for occasional procedures, as needed)
Marquam Hill (for occasional procedures, as needed)

Clinical Responsibilities:

• Provide independent, evidence-based medical care to BMT patients in an outpatient setting.
• Collaborate effectively with attending physicians to communicate clinical findings, treatment plans, and urgent issues.
• Evaluate patients for disease recurrence and order appropriate imaging and laboratory tests based on clinical indicators.
• Assist with managing immunosuppression, supportive care medications, and symptom control.
• Monitor patients for GVHD, infection, cytopenias, and other transplant related complications.
• Safely perform procedures after appropriate orientation and credentialing.
• Provide infusion unit support, including the management of infusion reactions and urgent evaluations.
• Educate patients and caregivers on transplant processes, medications, symptoms management, infections, and post-transplant expectations.
• Coordinate with inpatients teams and nursing staff on direct admissions and hospital discharge follow ups.
• Maintain accurate and comprehensive documentation of all clinical encounters in accordance with OHSU standards.
• Collaborate with multidisciplinary teams—including nursing, palliative care, social work, and allied health—to provide patient-centered care.
• Manage oncology clinical trial patients in accordance with protocol guidelines, ensuring documentation of adverse events and timely communication with study teams.
• After appropriate proctoring, APPs will independently perform procedures. Procedures include but are not limited to: Bone Marrow Biopsy and Aspiration, Lumbar Puncture, IT chemotherapy, Omaya, Skin Biopsy.

General responsibilities:

All APPs are expected to maintain professional standards in all aspects of role performance, including consistent participation in unit and departmental meetings, routine monitoring and timely response to organizational communications (e.g., email), and adherence to established deadlines for institutional and departmental requirements.

Required Qualifications

Experience:

Minimum of 2 years APP experience in hematology/oncology. Completion of an APP Fellowship in Hematology/Oncology may substitute for two years of the required experience.

Education:

Successful completion of a Master’s Degree in Nursing or Master of Science in Nursing in a recognized APRN specialty (Practitioner), OR Successful completion of a Master’s Degree in Physician Assistant/Associate Studies from an accredited program.

Registrations, Certifications and/or Licenses:

Current RN and APRN licensure with the Oregon Board of Nursing. Licensure with Washington Board of Nursing (within 90 days of start date). Current ANCC (or equivalent) Certification. DEA Certification OR Current PA licensure with the Oregon Medical Board. PA licensure with the Washington Medical Board (within 90 days of start date). Current NCCPA Certification. DEA Certification.

Knowledge, Skills & Abilities:

• Strong clinical assessment, diagnostic reasoning, and patient management skills.
• Ability to prioritize, organize, and manage multiple tasks in a fast-paced, high-acuity environment.
• Effective communication skills, including clear, concise, and professional interaction with patients, families, and multidisciplinary teams.
• Ability to collaborate across disciplines and contribute to coordinated, team-based care.
• Sound clinical judgment and decision-making, including the ability to recognize and respond to clinical deterioration.
• Proficiency in documentation and use of electronic health record (EHR) systems.
• Ability to adapt to changing clinical demands, workflows, and patient volumes.
• Strong attention to detail and commitment to patient safety and quality care.
• Ability to provide education to patients, families, and learners (e.g., students, trainees, new APPs).
• Demonstrated professionalism, accountability, and reliability in meeting responsibilities and deadlines.
• Ability to maintain confidentiality and exercise discretion in handling sensitive information.
• Commitment to continuous learning and adherence to institutional policies, standards, and evidence-based practice.

Physical Demands:

Must be able to travel between floors and clinical offices.

Preferred Qualifications

• Current BLS or ACLS.
• Experience working with pre and post BMT patients.
• Experience working with clinical trials and experimental therapeutics.
• Experience with EPIC and Beacon.
• Hematology/Oncology Certification.
• Advanced clinical assessment and diagnostic reasoning in complex hematologic/oncologic malignancies and BMT patients.
• Ability to recognize and manage oncologic, chemotherapy, biotherapy management, and cellular therapy-related complications, including monitoring and toxicity management.
• Strong clinical judgment and timely escalation of care for acutely ill and immunocompromised patients.
• Proficiency performing necessary procedures (e.g. bone marrow biopsies and aspirates, lumbar punctures, administration of chemotherapy).
• Effective communication with patients, families, and multidisciplinary teams, including complex care discussions and transitions of care.
• Ability to collaborate across specialties (e.g., oncology, infectious disease, critical care, pharmacy, case management) to coordinate comprehensive care.
• Proficiency in EHR documentation, order entry, and utilization of oncology-specific treatment plans and pathways.
• Ability to educate patients, families, and learners on complex disease processes, treatments, and supportive care needs.
• Strong attention to detail and adherence to safety standards related to high-risk therapies and immunocompromised populations.
• Demonstrated adaptability in managing evolving therapies, protocols, and clinical workflows in pre and post BMT care.
• Commitment to evidence-based practice, regulatory standards (e.g., FACT, CoC), and continuous professional development.
