Nurse Practitioner Compensation: $105,000 - $125,000 a year, plus a $5,000 signing bonus and RVU bonuses

Our Skilled Nursing Facilities in the Lima and Paulding, OH area are looking for a Nurse Practitioner to join a facility-based team of medical providers caring for patients living in long-term care facilities.

Job Qualifications: Current, unrestricted NP license in the state of Ohio Board certification DEA licensure Clinical background in adult, family or geriatric specialties Experience working in a long-term care setting

Responsibilities include but are not limited to: Provide primary and preventative care for long-term care patients residing in skilled nursing facilities Manage residents with an acute change in condition Advanced care planning and chronic care management Order, review and follow up on treatments and diagnostic testing Communicate regularly with nursing home staff and collaborate with the primary care physician
