Jackson Behavioral Health Hospital (JBHH) is a three-story, 180,000-square-foot facility committed to the highest quality, treatment, and service. Opened in 1999, JBHH consolidates evaluation, clinical treatment, teaching and research, to offer a full continuum of care for children, adolescents, adults and elderly, including crisis stabilization, inpatient hospitalization, and outpatient programs. In association with the University of Miami Leonard M. Miller School of Medicine's Department of Psychiatry, JBHH offers specialized treatment for numerous conditions including depression and mood disorders, schizophrenia, suicidal ideation or behavior, anxiety and panic disorders, drug and alcohol abuse and Alzheimer's disease. JBHH also specializes in criminal justice mental health services.

Summary

The Mental Health Intake Specialist is responsible for conducting comprehensive assessments, coordinating care, and developing individualized service plans to support the behavioral health, social, and medical needs of patients. This role ensures timely access to services, facilitates continuity of care, and promotes recovery through collaboration with interdisciplinary teams and community resources. Depending on unit-specific assignment, the incumbent may focus on initial evaluations and admission processes, or provide ongoing support and discharge planning to ensure successful community reintegration.

Responsibilities

• Conduct comprehensive clinical and psychosocial assessments to determine patient needs, strengths, barriers, and available resources.
• Identify and evaluate the social, emotional, and behavioral health needs of individuals using appropriate assessment tools and clinical judgment.
• Collaborate with patients, families, and interdisciplinary treatment teams to develop individualized plans of care tailored to the unique needs of each client.
• Monitor and update care or service plans to ensure appropriate treatment progression and effective care coordination.
• Provide patient education on available services, treatment options, medications, recovery strategies, and rights and responsibilities.
• Promote a Recovery-Oriented System of Care (ROSC) by supporting wellness, community integration, and long-term recovery goals.
• Ensure all required documentation—including assessments, service plans, progress notes, and outcome measures—is completed accurately and in a timely manner in compliance with regulatory standards.
• Coordinate with community agencies, referral sources, and support systems to ensure continuity of care and access to external resources.
• Respond to emergency situations and participate in crisis intervention as required.

• Performs all other related job duties as assigned.

Serve as the primary point of contact for patients and families throughout the care continuum. Certify the criteria for case management is met. Utilize the Critical Time Intervention (CTI) model to support successful reintegration into the community. Facilitate access to internal and external resources such as housing, employment services, financial assistance, and peer recovery support. Initiate and lead enhanced discharge planning from admission through transition, including confirming outpatient appointments, medication access, housing arrangements, and transportation. Educate patients and families about ongoing care, crisis resources, and relapse prevention strategies. Conduct follow-up outreach post-discharge to confirm engagement in care, identify barriers, and support adherence to treatment plans. Ensure warm handoffs to community providers and track patient outcomes such as appointment attendance and readmission rates. Utilize the High-Fidelity Wraparound process to assist designated individuals, youth and families with multiple and complex needs, through collaboration, team-based approach, strength-based, natural and formal support, person-centered approach, cultural sensitivity and community partnership.

Experience

Generally requires 1 to 3 years of related experience.

Education

Bachelor's degree in related field is required.

Skill

Credentials

American Heart Association and any additional applicable life support certification for Healthcare Providers may be required at the unit level upon hire with at least 6 months validity and maintenance at JHS for the duration of employment.

AHA Basic Life Support (BLS) with 6 months validity

Physical Requirements

Job function requires extended standing or walking. Must be able to carry objects weighing 21 to 50 pounds. Able to lift or move objects weighing 51 to 100 pounds with the use of appropriate devices. Jobs in this group require frequent visual acuity to perform activities related to extended use of computers. Environmental Conditions

Jobs in this group are required to function in a fast paced environment with occasional high pressure or emergent and stressful situations. Function is subject to inside environmental conditions, with occasional outdoor exposures. Exposure to various environments such as: communicable diseases, toxic substances, medicinal preparations and other conditions common to a hospital and medical office environment. May also be exposed to needle sticks, airborne infections, medical gases, X-Ray, chemical exposures and other potential hazards. Must wear Personal Protective Equipment (PPE) when exposed to infectious/clinical hospital environment.
