Job Description

Under the direct supervision of a registered pharmacist and following established policies and procedures, the Medication History Pharmacy Technician 1 provides technical support to pharmacists and clinicians to ensure safe and effective transitions in care at admission and discharge. Using a variety of strategies appropriate to the patient, family, and situation, the technician obtains, updates, and documents an accurate medication list in the patient’s electronic health record (EHR) by conducting patient interviews and using available electronic tools, including external prescription history and the hospital medical record system. • Update patient allergy history and preferred pharmacy information in the EHR to support accurate medication reconciliation.

  • Utilize all necessary sources—patient, family, hospital transfer orders, nursing and long-term care facility records, community pharmacies, and insurance fill history—to ensure the medication list is complete and correct.
  • Comply with all documentation requirements, including those for controlled substances and patient consultations, and follow supervisor notification procedures for dispensing incidents or PHI disclosures.
