MAT COORDINATOR/ LICENSED PRACTICAL NURSE (RN) POSITION SUMMARY:

The Medication Assisted Treatment Licensed Practical Nurse works as part of a team with the MAT Case Manger to manage all aspects of patient care for incarcerated individuals receiving Medication Assisted Treatment for Opioid Use Disorder and deliver a recovery-oriented system of care.

MAT COORDINATOR / LICENSED PRACTICAL NURSE (LPN) MINIMUM REQUIREMENTS:

Must have and maintain current licensure as a Licensed Practical Nurse in Delaware.
Medication Inventory Management
Initial and ongoing SUD (substance use disorder) assessment and evaluation
Patient Care Coordination
Collaborate with community treatment providers and social support services
Health Promotion
Comprehensive transitional care and re-entry planning
Attend meeting and trainings as required
Dispense MAT Medications
