Finger Lakes Health is seeking a compassionate and dedicated full-time Licensed Practical Nurse (LPN) to join our team at LifeCare in Seneca Falls. The LPN provides high-quality, patient-centered care in a practice setting while supporting our mission to improve the health and well-being of the communities we serve.

Our providers see patients of all ages from newborns to geriatrics. All care is provided by highly credentialed physicians Dr. Grace Freier, Dr. Elizabeth Osborn, Dr. Andrew Reese and Dr. Philipp Wirth. Well respected and experienced nurse practitioners Sarah D'Acquisto, MSN, AGPCNP-C, Camerin Scaglione MSN, RN, Arielle Hagner, PNP-C, Crystal Lane, AGPCNP-C, Shannon Picchi, AGPCNP-C as well as physician assistant David Stobie, PA-C.

Responsibilities:

• Provide direct nursing care to residents under the supervision of an RN
• Administer medications and treatments per physician orders and facility policies
• Observe, document, and report changes in residents’ conditions
• Collaborate with the interdisciplinary care team to support individualized care plans
• Ensure compliance with state, federal, and Finger Lakes Health regulations
• Maintain accurate, timely documentation in the electronic medical record
• Promote a safe, respectful, and compassionate environment for residents and families

Qualifications:

• Current New York State LPN license in good standing
• Strong clinical judgment, communication, and organizational skills
• Ability to work effectively as part of a team
• Commitment to resident-focused, compassionate care
