The LPN is prepared to function as a healthcare team member by exercising sound nursing judgment based on preparation, knowledge, experience in nursing, and competency. Under the direction and supervision of the Registered Nurse and the physicians, the LPN plans, implements and evaluates care and effectively addresses the individual needs and priorities consistent with the patient's cultural beliefs and values and implements select nursing actions within the LPNs scope of practice.

Job Duties

• Interacts with co-workers, patients, their family/designated care partner(s), and other staff consistent with the values of Jefferson.

• Contributes to monitoring the of nursing quality outcomes to continuously improve the quality and safety of patient care.
• An LPN shall communicate if the patient’s condition deteriorates or if there is a significant change in condition, the patient is not responding to therapy, the patient becomes unstable or the patient needs immediate assistance.
• Work closely with the registered nurse to assure the implementation of safe, quality care.

Minimum Qualifications

• Technical School Diploma Graduate of an accredited School of Practical Nursing and passed the Pennsylvania Board Association for Licensed Practical Nurse.
• Patient-and Family-Centered Practice Knowledge of how to convey respect and collaborate with each patient, family and/or designated care partner in care and care planning.
• Cultural Awareness Knowledge of people with diverse backgrounds and the ability to convey respect and connect effectively.
• Effective Communication Understanding of effective communication concepts, tools and techniques.
• Patient Evaluation and Care Knowledge of disorders and injuries of patients.
• Medication Administration and Prescription Knowledge of various medications used to treat patients.
• Discharge Planning Knowledge of the patient's post-discharge needs.
• Health Information Documentation Knowledge of the relevance of health information and appropriate documentation methods.
• BLS - Basic Life Support - American Heart Association.
• LPN - Licensed Practical Nurse_PA - State of Pennsylvania or LPN - Licensed Practical Nurse_NJ - State of New Jersey.

Physical Demands

Lift and carry 40-50 lbs. Examples: Push/pull patients on bed, stretcher (requires 29 lbs. push force), lateral transfers up to 50 lbs. of the patient's weight. Frequent to continuous standing/walking. Patient transporters can walk 8-10 miles per shift.

Salary Range

$27.58 to $39.30 Hourly. The actual hiring rate will be determined based on candidate experience, skills and qualifications.
